If you are a new patient to our office, please click on the below button which contain our new patient form that will need to be filled out when you arrive at the Downtown Dental office. Either filling and securely submitting online or printing, filling them by hand and bringing them with you, will allow us to attend to you more quickly than completing them on your arrival.
